Supporting Primary Care Networks

We also provide support and guidance to our members as they continue their development of local Primary Care Networks as part of the Government’s GP Contract Reform to implement the NHS Long Term Plan.

Primary Care Networks are groups of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local populations. We continue to provide support and guidance to assist our Primary Care Network leads and Clinical Directors as they work together across the Island.
